Coordination of Services Plan
For this paper I will describe three of the most prevalent social problems in Lincoln NE, and list the types of services that someone who is dealing with these issues might need.
I will also create a coordination of services plan and provide guidelines for case managers and other staff members in the agency to follow so that they are able to be successful with referring clients to these services.

Three Most Prevalent Social Problems in Lincoln NE
In my opinion the three most prevalent social problems in Lincoln NE, are homelessness, lack of healthcare and substance abuse. The types of services that an individual who is homeless might need are shelter, food, laundry, personal hygiene and employment. In Lincoln there is a shelter called the People’s City Mission that provides all of these services to the guests staying there. The types of services that a person who cannot afford healthcare might need are a physical by a medical professional, an eye exam and a dental exam, if it’s a woman she will also need her yearly pelvic exam.
The People’s City Mission has a medical clinic and is able to offer medical, dental, optical, and behavioral health services for low-income, uninsured residents of Lincoln and Lancaster County (Complete Guide to Human Service Resources, 2013). The types of services someone might need who have problems with substance abuse are treatment, HIV testing if they use needles, and mental health counseling if they are suffering from a mental disability. In Lincoln there are several facilities for treatment of substance abuse and several work on a sliding fee scale for those who qualify. Just to name a few there is Associates in Counseling and Treatment which provides substance abuse evaluation and treatment, and dual diagnosis counseling.
