Referral
The client is a 44-year-old, African-American female. She is feeling depressed due to family issues. Her eldest daughter just had her first baby in Tennessee and her husband is overseas. Her mother is currently blind in one eye and she just recently had a stroke. Mom wants to move back to Tennessee to help out with her mother, and her grandson, but her middle daughter is in the custody of the state for violence towards her. The client and her daughter have been trying to start counseling, but she is not complying with services. She wants to move back to Tennessee really soon, but does not want to leave her child behind. She also does not want to uproot her son, and leave the home her and her boyfriend just purchased. The client reports a loss of appetite and missing church frequently. She feels as if she does not want to do anything because her issues are becoming overwhelming, and feels as if she is drowning in them.
Demographics
The client is a 44-year-old, African-American female. She has never been married but she has three children. Her eldest is her 25-year-old daughter, 17-year-old daughter who is in custody of the state due to her violence towards her and her 14-year-old son. She is currently living with her boyfriend and her 14-year-old son. She was born in Tennessee and moved to Ohio in 2007.
History/Present
She was adopted as a young child. She continues to have a good relationship with her adoptive mother. She states that she was exceedingly close to her adoptive father, but he is deceased. The client had a sister who was the biological child of her adoptive parents. She admits that they did not have a very good relationship because her adoptive mother tended to side with her own biological daughter instead.
