Goal #1: To Reduce the percentage of homeless individuals and families in need of health care and other social services assistance.
Objective 1: Reduce by 25% number of homeless in need of immediate medical attention and other social service needs (September – March).
Outreach will go to areas frequented by the homeless to conduct one-on-one interviews.

Objective 2: Increase by 30% number of referrals and follow up services being handled (October – May).
Interns will assist Social workers to ensure that all clients are keeping appointments with doctors, housing authority/public housing and education & training.

Objective 3: Reduce by 15% the number of homeless in need of permanent housing (September-September of following year).
Collaboration between Health Care for the Homeless and Housing Authority/Public Housing to facilitate providing affordable housing for homeless families and individuals
Collaboration between HCH and Habitat for Humanity to facilitate the building of affordable housing for the homeless.
The outcome will be measured by statistical housing data derived from Habitat for Humanity and Housing Authority/Public Housing.
Goal # 2: To increase the number of medical professionals i.e. physicians and nurse practitioners and expand services to meet the demand of the homeless population.
Objective 1: Increase medical staff by 5% by September 2014.
