Safety policy and procedure violation.

VIOLATION LEVELS
1. Any violation of safety policy or procedure that resulted in serious physical injury or death to another associate or bystander.
2. Any violation of safety policy or procedure that results in an injury to the offending associate, to another associate or bystander, or could have resulted in serious physical injury or death to any associate or bystander (critical near miss).
3. Any violation of safety policy or procedure that results in moderate property damage or could have resulted in injury to any associate or bystander.
4. Any violation of safety policy or procedure that did not result in injury, potential injury or resulted in minor property damage (non critical near miss).

DISCIPLINARY LEVELS
1. Any violation of safety policy or procedure that resulted in a level one violation will result in termination
2. Any violation of safety policy or procedure that results in a level two violation will result in termination.
3. Any violation of safety policy or procedure that results in a level three violation will result in one of the following actions.
a. Level three with reckless intention will result in immediate drug test, and written warning.
b. Level three with wanton intention will result in immediate drug test, and suspension.
c. Level three with knowing intention will result in termination.
d. Level three with intentional intention will result in termination.

4. Any violation of safety policy or procedure that results in a level four violation will result in one of the following actions.
a. Level four with reckless will result in a verbal warning
b. Level four with wanton will result in a written warning
c. Level four with known intention will result in suspension
d. Level four with intentional will result in termination
