Statement

On Wednesday, March 6, 2013 I reported to work on time and worked on my regular assigned machine. About 9 -9:30 am I was in the middle of a set up and as I was setting my knives section like I always do, I attempted to turn the rolls and as I started to turn the dial the upper right side of my back immediately felt pain. I stopped doing what I was doing and tried to stretch out a bit, however, every movement caused pain. I went to my supervisor Marco Fuentes and explained what had happened. He then asked me if I was already injured or hurt prior to the pain. I told him that I had a stiff neck from sleeping the wrong way the night before and just now aggravated my back. I then asked him what I should do, go to the company clinic or my doctor to be examined. Marcos stated that he did not want to make that decision and said he was going to ask upper management. He returned and told me to clock out and make sure I return with a doctor’s release. So I did in fact leave and immediately called the doctor to see how soon I could be seen. I was able to get an appointment on that same day. I was examined and no major damage was done. He prescribed medication and instructed me to do light stretches and rest to let it heal. So he told me to take a couple days away from the work that initially aggravated the muscle. The next morning before my regular shift began I called Marcos and left a voicemail telling him that I would be out Thursday and Friday per my doctors instructions and I would be returning on Monday with the doctors release. Monday came, and feeling much better I began my shift as usual. I gave Marcos the paperwork from the doctor and went on with my day. On Tuesday I began my shift as usual. It was not until near the end of my shift Marcos came to me and asked me If I went to the emergency or in the case as with Kaiser, the Urgent Care Center. I let him know that when I called I was able to grab a cancellation time slot to see the doctor so there was no need...
