MCMG 312 Davenport University John Smith, 58 year old male who has recently suffered a new onset stroke that has left him with left sided weakness. Mr. Smith condition requires rehabilitation of his left side due to temporary paralysis. Mr. Smith major medical problems are listed as high blood pressure. Mr. Smith lives alone spouse passed away 2 years ago, has 2 children that he is estranged from and lives in different states. Given the circumstances, the Case Manager at the hospital believes that Mr. Smith would benefit from care in a rehabilitation program. Due to his left sided weakness, he would have to build strength to that side. Mr. Smith currently lives alone and has no immediate family that is available for help. Because of his left sided weakness it is has been determine that Mr. Smith would benefit from an in-patient rehabilitation until he is able to care for himself and has functioning mobility of left side. Mr. Smith will benefit from the three phases of post stroke care Acute and early rehabilitation, Ongoing rehabilitation, Living with stroke in the community. Plans have discussed over phone with his two children. Mr. Smith feels that he does not want to disrupt their families and is ok with going to a facility for rehabilitation. The acute episode starts at onset of stroke and involves the immediate diagnosis and management of the causes and consequences of the stroke (NSF, 2006). Mr. Smith has been evaluated by the Stroke/ Heart team within the hospital and it has been determine that there is no long term damage to his health and with proper care for his high blood pressure and rehabilitation he can live a fully functional life. Ongoing rehabilitation aims to improve function and/or prevent deterioration of function to bring about the highest level of independence, physically, psychologically, socially and economically.
