Instructions:
1. Please submit a valid Photo ID proof (Like PAN Card, Driving License, voters ID) along with a copy of this mail to the Diagnostic Centre
2. Please carry your original coupon (to be submitted to the centre), a copy of the health card (to be submitted to the test centre)
3. Fasting has to be followed for at least 12 hours before the test. Plain water may be consumed during this period. No Tea / Coffee to be consumed in the morning.
4. The original reports can be collected by you from the diagnostic centre
