The form is filled out by both the supervisor and the employee and is a reflection of the employee’s job performance. Honesty and integrity are encouraged by both supervisor and employee on this form for the accuracy of the employment file.

PERFORMANCE RATINGS Using the scale below check the box that applies/describes the employee’s job performance.
EXCEPTIONAL: Contributions to the company are excellent and recognized by others. Employee is consistently exceeding expectations. Employee is an above average producer and goes above and beyond to satisfy customers.
HIGHLY EFFECTIVE: Most performance objectives exceed expectations. Employee has an impact and increases business. The employee makes notable contributions to the department.
EFFECTIVE: Performance is competent and is right at level of expectations for job. Makes a partial contribution to the department.
IMPROVEMENT REQUIRED: Performance is below expectations on a few of job requirements and responsibilities. A performance improvement plan should be implemented.
UNSATISFACTORY: Performance is below expectations on several critical job requirements and responsibilities. If there is no improvement probation will be necessary. A performance improvement plan must be in place immediately.
